Jot Forms
I've just signed up for a free account and investigated JotForm; it looks like a very useful tool.
JotForm is a simple online tool for creating online forms of various kinds.
There are a number of pre-configured forms that managers/administrators/practitioners/learners can create and customize or they can create a blank one and add what ever features required from the Toolbox, just by dragging them onto the workspace.
Once all the required features are on the form, users can edit the properties by clicking on them in turn. I found it quite intuitive.
JotForm can be used for a whole range of things and has some very powerful features.
As well as adding images, password fields and captcha from the 'Power Tools' section, users can also add payment features (via PayPal for example), to enable users to charge for services.
The blank form creation, I would suggest, is great for creating eLearning materials and/or eAssessments.
Users can...
- add images,
- text
- by dragging in an html element from the Power Tools, even paste in code so that YouTube videos and/or other rich media can be embedded and
- add a few questions, or a text box for learners to write in
When the forms are completed, those filling in the forms just click on the submit button, and their answers will be sent to the email address (used to set up the account) or stored online.
Apart from creating e-learning activities, JotForm could be used to get feedback and/or to do a range of research tasks.
The resulting forms can also be embedded into websites, blogs, eXe documents, emails etc.
JotForm has a free option which allows for up to 3 forms a month and 100 responses, which should be enough for the average practitioner/learner.
